Event Title: Teaching an Old FMEA New Tricks (Presented at the 2016 World Conference on Quality and Improvement)

Topic Description:
Potential Failure Mode and Effects Analysis, or the FMEA, is a popular risk assessment tool used to manage potential risk in processes and designs. Not surprisingly, there may be failure modes associated with the activity of conducting an FMEA. Accurate results require accurate input rankings; however, complete and accurate information is not always available. When FMEA input rankings are based on collective intelligence that are skill-based and/or sometimes subjective, there needs to be a method of assessing the sensitivity of prioritized risk values associated with ranking uncertainty. This presentation will describe several potential failure modes associated with the activity of conducting an FMEA along with a simple method to assess the sensitivity of the Risk Priority Number to input rankings.
Learning Objectives:
• Learn potential failure modes associated with conducting the FMEA
• Understand why some RPN values are more sensitive to error than others
• Learn how to perform sensitivity analysis of RPN values for better decision making
• Learn how to update the standard FMEA template in order to measure marginal risk values


Program Speaker:
Eugene Bukowski is a senior engineering manager at General Electric Healthcare, Ambassador Medical in Noblesville, Indiana. His engineering team is responsible for the compliant refurbishment of ultrasound medical devices. Bukowski has more than 30 years of combined experience in the computer, automotive, health insurance, healthcare and management consulting industries, including IBM, General Motors, United Healthcare, the George Group, and Accenture. His industry experience includes functional assignments in product design and development, engineering management, large computer system sales and marketing, marketing strategy and business intelligence, finance capital management, corporate quality and continuous improvement, operations excellence, and business transformation. He is a Senior member of the ASQ, a Lean Six Sigma Master Black Belt and a Shainin® Red X® Master. He has a degree in electrical engineering from Purdue University, a master’s degree in electrical engineering from Duke University, and a degree in manufacturing management from Kettering University. Bukowski has published 10 inventions, six of which were awarded patents.

Poka-Yoke

In the 2016 ASQ World Conference Recap email issued June 1, 2016, the Quality tool of the week is Mistake Proofing, or Poka-Yoke.  The full article is available on the ASQ National website click here.

Mistake–Proofing Procedure

  1. Obtain or create a flowchart of the process. Review each step, thinking about where and when human errors are likely to occur.
  2. For each potential error, work back through the process to find its source.
  3. For each error, think of potential ways to make it impossible for the error to occur. Consider:
    • Elimination—eliminating the step that causes the error.
    • Replacement—replacing the step with an error-proof one.
    • Facilitation—making the correct action far easier than the error.
  4. If you cannot make it impossible for the error to occur, think of ways to detect the error and minimize its effects. Consider inspection method, setting function and regulatory function.
  5. Choose the best mistake-proofing method or device for each error. Test it, then implement it. Three kinds of inspection methods provide rapid feedback:
    • Successive inspection is done at the next step of the process by the next worker.
    • Self-inspection means workers check their own work immediately after doing it.
    • Source inspection checks, before the process step takes place, that conditions are correct. Often it’s automatic and keeps the process from proceeding until conditions are right.

5s is a simple but highly effective set of techniques that remove waste from your work environment through better workplace organization, visual communication, and general cleanliness. This fun session will take a different view of the 5 pillars of 5s as defined as sort, straighten, shine, standardize, and sustain. This topic will focus on sustaining results through Standardizing and Visual Management. Sustaining 5S does result in cost savings.

Program Speaker:
Leigh Ann Schildmeier is currently the Operational Excellence Leader at Nice-Pak in Mooresville. She also is the president and founder of the consulting firm, Park Avenue Solutions. She has extensive experience in Lean practices, and a Six Sigma Black Belt, she has worked for over 15 years in the manufacturing and pharmaceutical industries.

Leigh Ann has an engineering degree from Purdue University and MBA from Anderson University. She is a certified Lean Practitioner and Six Sigma Black Belt.

Leigh Ann has remained active in her hometown of Anderson by serving on the Advisory Board for Purdue Anderson College of Technology and the AHS D26 Career Center’s Manufacturing Advisory Board. She currently serves as the Chair for the ASQ Section 0904 East Central Indiana as well as ASQ Deputy Regional Director. Leigh Ann is married with a step-child and two Beagles in Fishers. She is a foodie, avid golfer, car enthusiast, Reds Baseball fan, and a loyal Purdue Boilermaker.

ASQ Certification Examination Changes

In Last Friday’s Fast Facts, ASQ National shared that effective September 28, 2016 ALL certification examinations will move to Computer Based Testing.  Computer Based Testing will commence with the October 2016 test cycle.

Benefits of the change include an increase of annual administrations from two to six; additional days available for testing from four to 102; more testing location from 350 to 8,000; convenience of retesting reduced from six months to two months; and exam results in zero to four days instead of 10.

The examinations will be available at Prometric testing sites.

Event Title: Traditional and Emerging Supplier Relationships: Synergies and Slippery Slopes

Event Description:
Supplier relationships are pivotal challenges for every organization. This holds for traditional manufacturing suppliers, and also for emerging suppliers of Cloud Computing, mobile devices, and other new technologies. Quality Assurance provides a host of approaches for traditional suppliers. But what if we are asked to cover suppliers of newer technologies? Do we have to start from scratch? It is a completely new game? No, not in the least. Many Quality Assurance approaches, controls, and solutions for traditional suppliers bridge time and technology. We are in better shape than we think. During this workshop Bob Eisenhut, a Quality Assurance professional in both traditional and emerging technologies, will look at both paradigms. He will show similarities, differences, points of synergy, and risk areas. Participants will get a chance for hands on work scenarios to engage directly in this activity. Anyone currently interested in supplier relationships who want to see connections to emerging technologies will be interested in this session.

Program Speaker:
Bob Eisenhut has 30 years of professional experience and has been an ASQ Certified Quality Engineer over 20 years. He has worked in various industries including manufacturing, airlines, advertising, software development, health care, and medical devices. His quality assurance work includes both quality engineering and software quality assurance. Bob has a BS in Mathematics and MS in Industrial Engineering from Purdue University. He has articles in publications including the SIAM Journal of Scientific and Statistical Computing, the journal of Medical Decision Making, and the Y2K Advisor. These covered areas of probability networks, analytical distribution approximations, systems optimization, and automated software testing. He is a trained lead auditor in ISO 13485 for the Medical Device Industry and is an auditor in information systems. Bob enjoys his quality roles and is involved in improvements in professional, volunteer, and social settings.

ASQ Social at Squealers

Last night Indy ASQ held a networking social at Squealers on 86th street in Indianapolis.  The event was well attended with a good mix of members from the Section Leadership Committee, members, new members and a few interested non-members.  We had some great conversations, great snacks and I saw a few business cards exchanging hands throughout the evening.   The Indy ASQ Socials are a great way to connect with the section and meet members in a relaxed informal atmosphere.  Our next social event is on May 4 at Chuy’s in Noblesville Indiana.  We hope to see you there! Improvement Project and Cost Savings

In a recently posted article and case study on ASQ National Website.  Janet Jacobsen describes how a DMADV project at Samarco Mining resulted in over 2M in annual savings.  The case study is available here.   The improvement project resulted in a 10% increase in profitability and ~89% reduction in non-quality production.
